What Is a MedsCheck?

A MedsCheck is a free, government-funded appointment with your pharmacist, created by the Ontario Ministry of Health for people managing multiple medications. In 20–30 minutes, we review every prescription drug, over-the-counter product, vitamin and supplement you take — checking for interactions, duplications, side effects and doses that no longer fit. You leave with a clear, updated personal medication record and practical answers to every question you’ve been meaning to ask.

If you take three or more medications for chronic conditions, you’re entitled to one full MedsCheck every year, plus follow-ups when things change — after a hospital stay, a new diagnosis, or a new prescription.

Senior Pharmacy Services in Toronto You Can Trust

Why Book Yours at Cloud Pharmacy

Our downtown Toronto pharmacists take the time most pharmacies can’t. Book your review in person at our Yonge–Dundas location, over the phone, or — if getting out is difficult — as a MedsCheck at Home. We coordinate directly with your doctor when changes make sense, sync your refills so everything is ready on the same day, and pair your review with free same-day delivery across the GTA. One conversation can genuinely make your medications safer, simpler and cheaper.

Medication Review Services We Offer

Annual MedsCheck

The core program: a yearly one-on-one review for anyone taking three or more prescription medications for chronic conditions. We assess every drug, supplement and over-the-counter product you use, then hand you a clean, complete medication record to keep and share.

Diabetes MedsCheck

Living with diabetes? You qualify for a dedicated annual review regardless of how many medications you take. We cover blood-sugar targets, insulin technique, monitoring devices and lifestyle questions — and connect it with our broader diabetes counseling support in the pharmacy.

MedsCheck at Home

If mobility, illness or distance makes visiting difficult, an eligible pharmacist review can happen right at your kitchen table. We come to you, review every medication in the house — including forgotten ones in the cabinet — and safely dispose of anything expired.

Follow-Up Reviews

Medications change — after a hospital discharge, a new diagnosis, or a new prescription from a specialist. Follow-up MedsCheck appointments keep your record current and catch problems early, exactly when the risk of interactions and confusion is at its highest.

Multiple-Medication Support

Juggling five, ten or more prescriptions? We simplify complex routines with weekly blister packs, synchronized refills and clear dosing schedules — so every pill is organized by day and time, and you or your caregiver always know exactly what to take.

Interaction & Side-Effect Checks

Feeling dizzy, foggy or “off” since a new prescription? We screen your full medication list for drug interactions, duplicate therapies and side-effect culprits, then work with your prescriber to adjust doses or switch to alternatives that agree with you better.

Refill Synchronization

Tired of pharmacy trips scattered across the month? We align all your prescriptions to one convenient refill date, coordinate renewals with your doctor before they run out, and deliver everything together — free, same-day, anywhere in the GTA.

Personal Medication Record

Every review ends with an up-to-date record of everything you take — names, doses, timing and purpose — in plain language. Keep it in your wallet, share it with specialists, and bring it to every appointment. In an emergency, it can be lifesaving.

A Small Conversation That Makes a Big Difference

Most medication problems don’t announce themselves — they build quietly: a duplicate therapy from two different specialists, a supplement that blunts a prescription, a dose that was right five years ago but isn’t today. A MedsCheck is how those problems get caught before they cause harm. It’s free, it takes half an hour, and patients routinely walk out with fewer pills, lower costs and far more confidence in their routine.

Book yours in person, by phone, or bundled with a free delivery — whatever is easiest for you.

Frequently Asked Questions

What is a MedsCheck and how much does it cost?

A MedsCheck is a structured, one-on-one medication review with a licensed pharmacist, funded by the Ontario government. It is completely free for eligible patients — the province pays the pharmacy directly, and there is never a charge to you.

Who qualifies for an annual MedsCheck?

Anyone with a valid Ontario health card who takes three or more prescription medications for chronic conditions qualifies for one full review every 12 months. People with diabetes qualify for a dedicated Diabetes MedsCheck regardless of how many medications they take.

How long does it take and what should I bring?

Plan for 20–30 minutes. Bring every medication you take — prescriptions, over-the-counter products, vitamins and supplements — or a complete list, plus your health card. If we already fill your prescriptions, we’ll have most of the picture ready before you arrive.

What is a MedsCheck at Home?

For patients who can’t easily visit the pharmacy due to physical or mental health conditions, an eligible pharmacist review can take place in your home. We review everything in your medicine cabinet, remove expired products safely, and leave you with an organized, current medication record.

Do I need to be a Cloud Pharmacy patient?

No — anyone can book. That said, the review is most powerful when we also manage your prescriptions, and transferring takes just 60 seconds online. Many patients book a MedsCheck as their first step when switching to us.

What happens after the review?

You receive an updated personal medication record listing everything you take, with doses and timing in plain language. If we spot anything your doctor should know about — an interaction, a dose concern, a simpler alternative — we contact them directly and follow up with you on the outcome.
