BHRT Compounding in Toronto: Custom Bioidentical Hormone Therapy
If you’ve hit a wall with commercial hormone therapy — a dose that’s too strong, a delivery method that doesn’t agree with you, or a formulation your body just doesn’t tolerate — compounded bioidentical hormone replacement therapy (BHRT) may be the next step to discuss with your prescriber. At Cloud Pharmacy Toronto is based compounding pharmacists work directly with physicians and nurse practitioners across the city to prepare BHRT formulations built around your bloodwork, your symptoms, and your prescription – not a one-size-fits-all dose.

What Is BHRT Compounding?
Bioidentical hormones are structurally identical to the hormones your body already produces — estradiol, estriol, progesterone, testosterone, and DHEA among them. Compounding is the process by which a licensed pharmacist takes a prescriber’s order and prepares that hormone at a specific strength, ratio, or in a delivery format that isn’t available as a mass-produced commercial product. That might mean a transdermal cream instead of a patch, a lower starting dose than any commercial tablet offers, or a combination of hormones in a single preparation.
It’s worth being upfront about something many patients don’t hear elsewhere: compounded BHRT isn’t automatically “safer” or “more natural” than commercial hormone therapy. The molecules are the same either way. Where compounding earns its place is when a commercial product doesn’t fit — the dose, the ratio, or the format your prescriber wants isn’t sold off the shelf. Our pharmacists will always be straight with you about whether compounding is the right fit for your prescription, or whether a commercial option would serve you just as well.
Who BHRT Compounding Helps
Toronto patients typically come to us for compounded BHRT during:
Perimenopause and menopause
Hot flashes, sleep disruption, mood changes, and vaginal dryness
Andropause (low testosterone)
Fatigue, low libido, and muscle loss in men
Thyroid and adrenal support
When standard dosing doesn't match lab-guided targets
Sensitivities to commercial excipients
Dyes, fillers, or preservatives that cause irritation
Post-hysterectomy or surgical menopause
Where a tailored titration schedule is needed
Every patient’s protocol is different, which is exactly the point of compounding: your prescriber sets the clinical direction, and we build the formulation around it.
How Our BHRT Compounding Process Works
- Your prescriber writes the order. Your physician or nurse practitioner determines your hormone needs based on bloodwork and symptoms, then sends us a compounding prescription by fax, e-fax, or transfer.
- Our pharmacists review and formulate. We confirm the formulation, base, and strength, and prepare your BHRT compound in-house, following Ontario College of Pharmacists (OCP) standards and NAPRA Model Standards for non-sterile compounding.
- You pick up or arrange delivery. Most BHRT compounds are ready within 24–72 hours. We offer in-store pickup as well as delivery across Toronto and the GTA.
- Ongoing follow-up. As your prescriber adjusts your protocol based on follow-up labs, we refill and reformulate accordingly — no need to start the process from scratch each time.

Formats We Prepare
Depending on your prescriber’s order, compounded BHRT can be prepared as:
- Transdermal creams and gels
- Oral capsules
- Sublingual troches and drops
- Vaginal preparations
The right format depends on how your body absorbs and metabolizes hormones, your symptoms, and your day-to-day routine — a conversation best had between you, your prescriber, and our compounding pharmacist.
Serving Toronto and the GTA
Hormone compounding in Ontario requires a higher level of pharmacy accreditation than most retail pharmacies carry. Non-sterile compounding is classified by NAPRA and the OCP into Levels A through C based on risk — and hormone preparations require Level C, the highest non-sterile designation. Ask any compounding pharmacy in Toronto what level they’re accredited for; a reputable one will tell you and provide documentation on request. Cloud Pharmacy Toronto is proud to serve patients and prescribers throughout downtown Toronto, North York, Etobicoke, Scarborough, and neighbouring communities including Mississauga and Vaughan, with in-store pickup and GTA-wide delivery.
