Who Pays for Prescriptions in Ontario?

Prescription costs in Ontario are covered in three main ways: public drug programs funded by the province, private insurance through your employer or school, and out-of-pocket payment. The public programs all operate under the Ontario Drug Benefit (ODB) umbrella, which covers seniors aged 65 and over, Ontario Works and ODSP recipients, residents of long-term care homes, patients receiving professional home care, and anyone enrolled in the Trillium Drug Program. Separately, OHIP+ covers most Ontarians aged 24 and under who don’t have a private plan.

Ontario Drug Benefit (ODB)

Ontario’s main public drug plan, covering roughly 5,000 medications. It includes seniors 65+, long-term care residents, home-care patients, and anyone on ODSP, Ontario Works or Trillium. Senior coverage begins automatically the month after your 65th birthday — no application needed.

Trillium Drug Program (TDP)

An income-based plan for any household spending a large share of income on prescriptions. You pay a deductible of about 4% of net income in quarterly instalments; after that, each prescription costs only a small co-pay. Apply any time — we help with the forms.

OHIP+ for Ages 24 & Under

Who qualifies for the Ontario Drug Benefit (ODB)?

You automatically qualify on the first day of the month after you turn 65. You also qualify at any age if you live in a long-term care home or a home for special care, receive professional home-care services, are enrolled in Ontario Works or ODSP, or are registered in the Trillium Drug Program.

How does the Trillium Drug Program work?

Trillium is income-based rather than age-based. Your household pays an annual deductible of about 4% of its combined after-tax income, divided into four quarterly instalments. Once the quarter’s deductible is met, each covered prescription costs no more than a small co-pay (currently around $2). It’s designed for anyone whose drug costs are high relative to their income — including self-employed people and those without workplace benefits.

What is OHIP+ and who is covered?

OHIP+ provides free coverage of more than 5,000 medications for everyone aged 24 and under who is covered by OHIP and does not have a private insurance plan. There’s no application — coverage is automatic when the pharmacy processes an eligible prescription with a valid Ontario health card.

What if my medication isn’t covered by the ODB formulary?

You have options. Your prescriber can apply to the Exceptional Access Program (EAP) for case-by-case coverage, and in many situations a covered generic or therapeutic alternative exists that works just as well. Can Cloud Pharmacy bill my private insurance directly?
